I’m a women’s health coach dedicated to helping midlife women feel energised, balanced, and confident again. After experiencing my own struggles with perimenopause, I realised how many women were being told to just “push through” their exhaustion and mood swings. That’s when I decided to create a more compassionate, science-backed approach to support women through this transition.

Through my signature programme, The Peri Protocol, and my workshops, I help women understand what’s really happening in their bodies, manage stress effectively, and make simple lifestyle shifts that restore energy and calm. As women enter midlife, many find themselves wondering why they suddenly feel so different.Sleep becomes fragile, energy disappears, emotions feel unpredictable and concentrating becomes harder than it used to be.

Mixed messages about symptoms and support can make this time in our lives even more difficult.

The Confusion Around Hormone Replacement Therapy

Hormone Replacement Therapy (HRT) is more commonly called Menopause Hormone Therapy (MHT). It has been talked about a lot in recent years and the dialogue is shifting. For decades we were told it is dangerous and should be avoided. But the pendulum has now swung and many tout it as the elixir of life. The truth is far more nuanced. For many women, MHT is safe, effective and can be incredibly helpful during peri and post menopause. But it isn’t the only piece of the puzzle. Understanding when it is worth considering MHT can help you make informed decisions about your health and wellbeing.

What is MHT and How Does It Work?

The hormones you take during MHT replace some of the hormones that naturally decline during perimenopause and menopause. For most women this involves oestrogen and is often combined with progesterone if they still have a uterus.

By restoring hormone levels to a more stable range, MHT can help regulate many of the systems in the body that depend on oestrogen, including temperature control, sleep regulation, brain function, and bone health. For some women, this can dramatically improve symptoms such as hot flushes, night sweats, sleep disruption, and mood instability.

If you’ve been struggling with symptoms that are affecting your quality of life, these may be signs it’s time to have a conversation with your doctor.

The 7 Signs You Might be Ready to Consider HRT during Perimenopause

1. Your Symptoms Are Affecting Your Quality of Life

Many women can expect to experience mild discomfort during perimenopause. It’s a big change in hormones so it’s unlikely that the transition will pass without you realising. Symptoms are often initially dismissed as stress or part of getting older. When your symptoms however are interfering with daily function, it is a good time to consider speaking to your doctor about MHT.

When fatigue makes it difficult to get through the day, when brain fog starts affecting your work, when you are experiencing constant hot flushes, or when anxiety feels unfamiliar, persistent, or scary, it may be a signal that hormonal changes are playing a bigger role than you realised. The goal is not to endure symptoms unnecessarily. If your quality of life is being affected, it is reasonable to explore options that might help.

2. Lifestyle Changes Aren’t Enough

Symptoms of perimenopause can often be moderated with lifestyle. You might have already made meaningful changes such as improving diet, exercising regularly, reducing alcohol, or implementing a sleep routine to get better sleep. These strategies can make a significant difference for many women.

But perimenopause is a biological transition, and sometimes lifestyle alone isn’t enough to fully manage the symptoms and your hormonal changes may need some additional support.

3. Your Sleep Has Completely Broken Down

Sleep disruption is one of the most debilitating perimenopause symptoms. During perimenopause, declining progesterone and fluctuating oestrogen can disrupt the brain’s sleep signals. At the same time, cortisol (the body’s stress hormone) can become more active at night. This combination often leads to the classic pattern many women experience: falling asleep easily but waking suddenly in the early hours of the morning with a racing mind.

If you are regularly waking at 2–4amy, experiencing night sweats, racing thoughts, and an inability to fall back asleep, it may be time to consider MHT. Long-term sleep deprivation impacts your metabolism, mood, brain health, and weight regulation. MHT can sometimes restore the hormonal signals that support good quality sleep.

4. You’re Experiencing Intense Mood Changes

Hormonal shifts can strongly affect the neurotransmitters that send the messages to our brain that determine how we feel. Symptoms of hormonal interruption of neurotransmitters may include anxiety, irritability, sudden mood swings, and loss of emotional resilience.

These changes can be particularly unsettling for women who have always felt emotionally steady and resilient. Many describe feeling more reactive, more sensitive, or less able to cope with stress than they used to be. Women often say "I just don’t feel like myself anymore” and recognising that hormones can influence these changes can be incredibly reassuring. Hormonal therapy may stabilise these fluctuations.

5. You’re Experiencing Significant Hot Flushes or Night Sweats

Hot flushes and night sweats are classic vasomotor symptoms. They occur when the brain’s temperature regulation system becomes more sensitive to small changes in body temperature. Oestrogen plays a role in stabilising this system so when levels fluctuate, the hypothalamus can overreact and trigger a sudden heat response. This inability to regulate body temperature may feel like you have a heater strapped to your stomach. Signs of this may include frequent daytime hot flushes, drenching night sweats, disrupted sleep, and feeling constantly overheated. For some women this happens occasionally. For others it can occur dozens of times a day.

These symptoms are one of the most evidence-based reasons for HRT.

6. You Have Early Bone Density Concerns

Declining oestrogen levels affects our bone health. Oestrogen helps regulate the process by which bone tissue is broken down and rebuilt. As Oestrogen declines, bone breakdown can occur more quickly than bone rebuilding, leading to a gradual loss of bone density. Risk factors include a family history of osteoporosis, having a small body frame, early menopause, and a history of fractures.

HRT can support bone protection during the transition years and is recommended for women who have low bone density. It is a good idea to get a bone density test when you are aware that you are in perimenopause as this gives you a baseline to track your bone density over time. Declining bone density leaves you extremely susceptible to falls and fractures which not only dramatically reduce your quality of life but can also reduce the longevity of life.

This is why bone health becomes a major focus of midlife health care. Hormone Therapy is an important part of ensuring bone density however strength building is also critical here.

7. You Feel Exhausted by Trying to Push Through

Many women try to power through perimenopause. After decades of caring for others, perimenopause often arrives during a demanding stage of life. Trying to push through significant symptoms while carrying this mental and emotional load can be exhausting.

When symptoms accumulate and you are dealing with fatigue, overwhelm, brain fog, and poor recovery from injury or illness, it may be time to consider MHT. You have permission to protect your wellbeing, you don’t need to suffer through perimenopause!

Choosing MHT

For some women, MHT can feel like a relief. Alongside lifestyle changes, iIt can be a way to restore stability and regain a sense of normality. For others, lifestyle strategies alone may provide enough support. Choosing MHT isn’t for every woman but it might be a good option for you to consider for your circumstances. There is no single “right” choice. The most important thing is that women feel informed, supported, and empowered to make decisions about their own health. Regardless of your choice, the goal is always the same: a better quality of life, better long term health, and feeling like yourself again. Lifestyle plays a really important part of health during perimenopause regardless of whether you choose MHT.

Who Should Not Be Taking MHT?

While MHT is safe and beneficial for many women, it is not suitable for everyone. Women with certain medical histories such as certain cancers or clotting disorders may need alternative approaches. This is why it is important to discuss your personal health history with a knowledgeable doctor before making a decision.

How to Support Your Changing Hormones

Even for women taking MHT, nutrition, sleep, movement and stress still matter enormously. Hormones are one piece of the puzzle. Supporting your body holistically is what helps women truly thrive through this transition.

Perimenopause is not simply a medical condition to survive — it is a natural transition in a woman’s life. With the right support, information, and care, it can also become a period of growth, strength, and renewed wellbeing. Whether you choose lifestyle support, hormone therapy, or a combination of both, the goal is the same: to feel well in your body and confident in this next stage of life.
